Report an ID being misused.

An ID presented by someone it does not belong to is a question about that person, not about the record. Medvetta can only act on what it can check, so the more of the following you have, the faster it resolves.

This page does not file the report.Medvetta has no public intake behind this page yet. Gather what is below and send it through Medvetta’s contact channel; nothing you type on this page is recorded anywhere.
What to gather
The ID exactly as it was given to you — nine digits, and whatever letters came with them.
Where you saw it: a badge, a website, a printed packet, a message, a business card.
The name the person gave, and the designation they claimed.
The date and, if you have it, the practice or address where it happened.
What happens next
Medvetta checks the ID against its own registry and against the record it points at.
The person who holds the record is told that their ID was presented by someone else — it is their identity being used, and they are the first to need to know.
Where a record turns out to have been claimed dishonestly, Medvetta closes it. A closed number is never re-issued.
A report is not a complaint to a licensing board, and Medvetta does not make one on your behalf. If what happened is a matter for the board that issued the license, that is a separate report, made directly to them.
